Who We Are
The Center for Community Practice (CCP), formerly known as the Center for Health and Behavioral Training (CHBT), has over 25 years of experience building capacity of the nation’s HIV/STD/HCV workforce. We are:
  • A public health community practice center of the Infectious Diseases Division at the University of Rochester Medical Center (URMC) in Rochester, New York;
  • A capacity-building center for state and jurisdictional health departments and their partners in the CDC national Capacity-Building Provider Network;
  • A collaborative partner of Monroe County Department of Health offering STD/HIV prevention and clinical management for the underserved community locally in the greater Rochester area;
  • The New York State designated STD Center of Excellence for clinical education, consultation and technical assistance;
  • The New York State designated Regional Training Center for comprehensive community-based HIV training and curriculum development for upstate New York;
  • The world’s first STD Project ECHO (Extension for Community Health Outcomes) through a partnership with the New York State Department of Health AIDS Institute
  • A sister agency to the Center for AIDS Research (CFAR) at URMC Infectious Diseases with collaborative partnership with world renowned medical and behavioral scientists and infectious diseases experts;
  • Faculties and physicians serving HIV and other infectious diseases patients at the Strong Memorial Hospital and the AIDS Center of the URMC;
  • Clinical providers, trainers, and capacity-building consultants with direct patient care experience and expertise in research, curriculum development, prevention counseling, coaching, training and education, and a real-world understanding of how to bridge science and practice through capacity building and training
